ALL children & youth participating in a youth program must complete:
- The Health/Permission form! A new form is required annually, starting July 1st. Every minor participating in a Yearly Meeting Program must have a completed form on file.
Participants in the Young Friends (high-school-age) program must also complete:
- The Young Friends Guidelines Affirmation! The Guidelines Affirmation requires youth and guardian signatures before the young person participates in a Young Friends event. This form does not need to be renewed annually or otherwise.
